Double-ended pusher: a shaped blade at one end for lifting cuticle out of the side walls, a pusher at the other for taking it back off the plate.
Two ends on one handle is the practical arrangement for a treatment where you switch between those two actions constantly. It also means one instrument through the autoclave instead of two.
The handle is knurled along its length, which is what keeps it from turning in wet gloves.

Care
Stainless steel, so it takes autoclaving and chemical disinfection between clients. Dry it properly before it goes back in the pouch: moisture left in a hinge is what causes staining and eventually stiffness, not the sterilisation itself.
